What fibre is

Dietary fibre is the structural part of plant foods that human digestive enzymes cannot break down. It passes through the small intestine largely intact and reaches the colon, where some of it is fermented by bacteria. Because it is not digested, it contributes almost nothing to blood glucose — and because of its physical properties, it changes how everything eaten with it is absorbed.

There are two broad types, and most plant foods contain both:

Soluble fibre dissolves in water to form a gel. It is found in oats and barley (as beta-glucan), legumes, apples, citrus, carrots, flaxseed, chia and psyllium. This is the type with the direct effect on glucose.

Insoluble fibre does not dissolve; it adds bulk and speeds transit. It is found in wheat bran, whole grains, nuts, seeds and the skins of vegetables and fruit. Its effect on glucose is indirect.

How soluble fibre steadies glucose

When soluble fibre meets water in the stomach and small intestine it thickens the contents into a gel. That gel:

  • slows stomach emptying, so carbohydrate reaches the small intestine more gradually;
  • slows the contact between digestive enzymes and starch, so starch is converted to glucose more slowly;
  • slows the movement of glucose to the intestinal wall, so it is absorbed over a longer period.

The result is the same total glucose absorbed over a lower, flatter, longer curve. A lower peak means less demand on insulin — the mechanism explained under insulin resistance — and less time spent in the range where damage accumulates.

Soluble fibre also binds bile acids in the gut, which is why oats, barley and psyllium lower LDL cholesterol — a second benefit relevant to people with insulin resistance.

How fermentation helps

In the colon, bacteria ferment soluble and some insoluble fibre into short-chain fatty acids (acetate, propionate and butyrate). These feed the cells lining the colon, reduce inflammation, and appear to improve insulin sensitivity and promote the release of gut hormones (including GLP-1) that increase insulin secretion and reduce appetite. This is one of the mechanisms by which higher fibre intake is linked to lower diabetes risk in large population studies, beyond the immediate meal effect.

What the evidence shows

Population studies consistently find that people who eat more fibre — particularly cereal fibre — have a lower risk of developing type 2 diabetes — and of prediabetes progressing. Each additional 10 grams a day is associated with a meaningful reduction.

Randomized trials in people with type 2 diabetes find that increasing fibre intake lowers fasting glucose and A1C modestly, with larger effects in people whose starting intake was low and when the fibre is soluble. Psyllium taken before meals has some of the most consistent results among supplements, reducing post-meal and fasting glucose across multiple trials.

The effect sizes are smaller than those of medication or substantial weight loss, but fibre has few downsides beyond initial bloating, lowers cholesterol at the same time, and comes with the rest of a better diet.

How much fibre

Recommendations for adults are about 25 grams a day for women and 38 for men, or 14 grams per 1,000 calories. Average intake in the United States, United Kingdom, Canada and Australia is roughly half of that.

Legumes are the most efficient source by a wide margin; a cup of lentils delivers more fibre than most people eat in a day.

Increasing fibre without discomfort

  • Raise intake gradually over two to three weeks; the gut bacteria need time to adjust.
  • Drink more water — fibre without water can worsen constipation.
  • Add rather than replace at first: beans to a soup, seeds to yoghurt, an extra vegetable at dinner.
  • Swap refined grains for intact ones: steel-cut or rolled oats for instant, barley for white rice, dense seeded bread for white.
  • Keep the skin on fruit and vegetables where edible.

Fibre supplements

Psyllium husk (the ingredient in Metamucil and generic equivalents), glucomannan, beta-glucan, inulin and methylcellulose are the common ones. Psyllium and glucomannan have the most direct evidence for glucose, typically taken 10–15 minutes before a meal with a full glass of water, at doses of 5–10 grams (psyllium) or 1–3 grams (glucomannan). They should be taken at least two hours apart from medications, whose absorption they can reduce, and they can cause choking if swallowed dry.

A supplement can be a practical way to add soluble fibre, particularly for people who struggle to eat enough legumes and whole grains. Frequently asked questions

Does fibre count as carbohydrate?

Fibre is chemically a carbohydrate and appears within "total carbohydrate" on nutrition labels, but it is not digested into glucose. Some people on carbohydrate counting subtract fibre from total carbohydrate ("net carbs"); this is reasonable for foods high in fibre but the effect is small for most foods.

Is a fibre supplement as good as fibre from food?

For the specific effect on post-meal glucose, psyllium before meals performs comparably to food fibre in trials. Food fibre brings vitamins, minerals, polyphenols and variety that a supplement does not, and it tends to displace refined carbohydrate, which a supplement does not. Can too much fibre be a problem?

Increasing fibre quickly causes bloating, gas and cramping; raising it gradually over a few weeks with plenty of water avoids most of this. Very high intakes can interfere with mineral absorption, and fibre supplements should be taken apart from medications.
