Gymnema sylvestre is an Indian climbing plant whose leaves contain gymnemic acids, which temporarily block sweet-taste receptors on the tongue. A small number of human trials — mostly old, small and without placebo controls — suggest extracts of 200–400 mg a day may lower fasting glucose and A1C in people with diabetes, but the evidence is too weak to rely on. It can add to the effect of diabetes medication and cause low blood sugar. The taste-blocking effect is real but lasts under an hour.
What gymnema is
Gymnema sylvestre is a woody climbing plant native to India, Africa and Australia, used in Ayurvedic medicine for centuries under the Hindi name gurmar — "sugar destroyer". The name refers to a striking property: chewing the leaves temporarily removes the ability to taste sweetness.
The active compounds are gymnemic acids, a group of molecules structurally similar to glucose. Supplements use leaf extracts standardized to gymnemic acid content, typically 25–75%.
How it appears to work
Two mechanisms are proposed:
- Blocking sweet-taste receptors on the tongue. Gymnemic acids bind to the receptors that detect sugar, so sweet food tastes of nothing. This effect is well documented and short-lived.
- Reducing glucose absorption in the gut and stimulating insulin release. Gymnemic acids may bind to receptors in the intestinal lining that would otherwise take up glucose, and laboratory studies suggest they may increase insulin secretion from the pancreas. Some animal studies report regeneration of insulin-producing cells; this has not been shown in humans.

The evidence has not advanced much in twenty years. Well-designed, placebo-controlled trials are largely absent, which is why authoritative sources describe the evidence as insufficient to draw conclusions.
Doses studied
Trials used 200–400 mg a day of an extract standardized to about 25% gymnemic acids, usually in divided doses before meals. For the taste effect, lozenges or a leaf infusion held in the mouth are used shortly before or after eating sweet food.
Side effects and safety
Reported side effects are mild — nausea and stomach discomfort. The main safety concern is hypoglycemia when combined with insulin or oral diabetes medication. A few case reports have linked gymnema to liver injury, though causation is uncertain. It has not been studied in pregnancy or breastfeeding and should be avoided in both.

Gymnema is a very common ingredient in blood sugar formulas, usually listed as "gymnema sylvestre leaf extract" at 100–400 mg and rarely with its gymnemic acid content stated. Can gymnema regenerate insulin-producing cells?
