Every sugary exposure starts an acid dip lasting 20–40 minutes regardless of size — a single biscuit and a whole cake cost similar time underwater. Which yields the least intuitive fact in dentistry: frequency beats quantity. Dessert with lunch is one attack; the same sugar grazed across an afternoon, or a sweet drink sipped for hours, is a tide that never goes out. Decay is what happens when the arithmetic of attacks and recoveries runs negative for months.

The stages

  1. White spot — chalky, matte patch, often at the gumline or between teeth: net mineral loss under an intact surface. Reversible with fluoride and habit repair.
  2. Enamel cavity — the surface breaks; a catch a dentist's probe finds. Fillable, small.
  3. Dentine decay — past the enamel into softer, tubuled dentine, decay widens and accelerates; sensitivity to sweet and cold appears. Fillable, larger.
  4. Pulp involvement — bacteria reach the nerve chamber: spontaneous ache, lingering hot/cold pain, night pain. Root canal treatment or extraction territory.
  5. Abscess — infection exits the root tip into bone: throbbing pain, swelling, sometimes fever. Urgent care; facial swelling or difficulty swallowing is emergency care.

Adults add a modern chapter: root decay — gum recession (see gum disease) exposes root surfaces that lack enamel entirely and decay faster, the signature cavity of later life and of dry mouths.

Toward decay: frequent sugars and sipped sweet or acidic drinks; dry mouth — the underrated one, from hundreds of common medications (antidepressants, antihistamines, blood-pressure drugs), mouth-breathing, and disease — because saliva is the entire repair service; deep fissures and crowding; exposed roots; appliances that trap plaque; reflux and vomiting (acid from within); and poverty of fluoride exposure. Diabetes contributes via glucose in saliva and dryness.

Toward repair: fluoride at the surface (paste, water, varnish), saliva flow (hydration, sugar-free gum), confined eating occasions, cheese and milk endings (calcium, phosphate, pH buffering), and plaque disrupted daily so acid is made next to the tooth for less time.

Prevention that actually works

  • Fluoride toothpaste, twice daily, spit don't rinse — the single most evidence-backed habit in dentistry (1,350–1,500 ppm for adults; a smear/pea for children with supervision).
  • Confine sugar to mealtimes; between meals, water. Treat juice, sports drinks, sweetened coffee and "health" smoothies as the dessert course they chemically are.

  • Sugar-free gum (especially xylitol-sweetened — see xylitol) after meals when brushing isn't possible.
  • Dental visits on the interval your risk earns — high-risk mouths gain from professional fluoride varnish; children's molars from sealants, which cut fissure decay dramatically.
  • Fix dry mouth — review medications, hydrate, use saliva substitutes; a dry mouth is a high-risk mouth whatever the diet.
  • Prescription-strength fluoride paste (2,800–5,000 ppm) for adults with active decay or exposed roots — ask, it exists.

Treatment, briefly

White spots: fluoride varnish and habits, sometimes resin infiltration. Cavities: fillings (composite bonded resin for most). Deep decay: root canal treatment preserves the tooth; extraction where it cannot. Modern dentistry also increasingly arrests selected decay without drilling (silver diamine fluoride, notably in children and root caries) — worth asking about. And symptom-free: the check-up interval exists because stages one and two — the cheap, painless stages — are found by probes and X-rays, never by feel.

Frequently asked questions

Why shouldn't I rinse after brushing?

Rinsing washes away the fluoride the paste just deposited — the after-brushing reservoir is much of the benefit. Spit out the excess and leave the film; if you like a rinse, use a fluoride mouthwash at a different time of day.

Should I brush right after sweets or acidic drinks?

Wait 30–60 minutes after acidic items (citrus, soda, wine): acid softens the enamel surface, and immediate brushing scrubs softened mineral away. Rinse with water or chew sugar-free gum meanwhile — the gum stimulates saliva, the repair fluid.

Are "sugar-free" drinks safe for teeth?

Safer for decay (no fermentable sugar) but many are acidic enough to erode enamel directly — a different, bacteria-free process. Sipped all day, diet sodas and flavored sparkling waters still wear teeth. Water and plain milk are the only truly indifferent drinks.

Can a cavity heal on its own?

Only at the earliest stage. A white-spot lesion — mineral lost beneath an intact surface — can remineralize with fluoride, cleaned-up habits and time. Once the surface cavitates, bacteria occupy a cave the body cannot resurface; from there dentistry is the only route, and the sooner the smaller.
