Tongkat ali (Eurycoma longifolia, "longjack") is a Southeast Asian root used for male vitality. A 2022 systematic review and meta-analysis of five randomized trials found a statistically significant rise in total testosterone with supplementation, most convincing in older men and men starting with low levels; typical doses were 100–400 mg per day of standardized water extract for 2–6 months. Erectile function is a different story: a meta-analysis of the two available trials found no significant improvement. Reported side effects are mild, but products in this category have repeated adulteration findings, and men with prostate or other hormone-sensitive cancers should avoid it.
What tongkat ali is
Tongkat ali is the root of Eurycoma longifolia, a slender tree of Malaysian and Indonesian forests, long used as a tonic for energy and male vitality (and, traditionally, for fever and malaria — its quassinoid compounds are genuinely bitter and biologically active). Supplements use water extracts of the root, commonly concentrated 100:1 and sometimes standardized to eurycomanone, the best-studied constituent.
How it appears to work
Proposed mechanisms, mostly from laboratory and animal work: increased release of free testosterone from its binding protein (SHBG), stimulation of Leydig-cell testosterone synthesis, and reduced cortisol. In humans, the observable facts are the hormone changes in the trials below; the mechanistic story remains partly inferred.
Evidence by use
Testosterone — the headline. A 2022 systematic review identified nine clinical studies and meta-analyzed five randomized trials: supplementation significantly increased total testosterone (a large pooled standardized effect, with wide confidence intervals reflecting small, heterogeneous trials). The rise was most consistent in older men and men with low baseline testosterone; several included trials used 200 mg/day of a standardized water extract for 1–6 months. Caveats worth their weight: the trials are small, some involve the manufacturer of the dominant branded extract, and "statistically significant rise" is not the same as "restored vitality" — symptom outcomes were secondary and mixed.

Libido and wellbeing. Several small trials report improved libido scores and sexual wellbeing, plausibly downstream of the hormonal effect in the men who respond.
Stress, mood, body composition, sperm quality, performance. One-or-two-small-studies territory across the board: directionally interesting, decision-grade nowhere.
Doses studied
100–400 mg per day of a standardized water extract, most commonly 200 mg/day, for 4 weeks to 6 months. Higher is not evidenced as better. Raw root powders and "complexes" without extract ratios are not what the trials used.
Safety
Trials report good tolerability: occasional restlessness, insomnia (some users find it stimulating — morning dosing helps), irritability and stomach upset. Specific cautions:
- Hormone-sensitive conditions — prostate or breast cancer: avoid, on mechanism.
- Diabetes and blood-pressure medication — animal data suggest glucose- and pressure-lowering activity; human data are thin, so monitor and tell your prescriber.
- Pregnancy and breastfeeding — avoid; no data.
- Quality and adulteration — this category has repeated regulator findings of undeclared drugs, and heavy-metal contamination has been reported in some Southeast Asian raw products. Third-party-tested, standardized extracts only.
- Long-term safety beyond ~6 months is simply unstudied.
Where it appears in supplements
As a stand-alone (usually 200–400 mg standardized extract) and as the marquee name in testosterone-booster blends — where it typically appears inside a proprietary blend at an undisclosed fraction of the studied dose. The gap between the trialled article and the marketed one is the recurring theme of our men's health product research.
Bottom line
For an older man, or one with confirmed borderline-low testosterone who has addressed sleep, weight and alcohol and wants a time-limited experiment his doctor knows about, tongkat ali at 200 mg/day of a standardized extract is one of the two defensible picks in this category (ashwagandha being the other, by a different route). For a young man with normal levels, or anyone whose real question is erections, the evidence says: different problem, different answer — start with low testosterone or erectile dysfunction and their look-alikes.
Frequently asked questions
How much does tongkat ali raise testosterone?
The meta-analysis reports a significant pooled rise without a single clean number, because trials measured different populations and doses; individual trials in older or low-testosterone men showed meaningful average increases, while effects in young healthy men were smaller or absent. Treat it as "can lift a low level toward normal," not "adds a fixed amount to any level."
Tongkat ali or ashwagandha?
Different bets. Ashwagandha's rise is a stress-pathway effect with better-replicated calm/sleep benefits attached; tongkat ali's evidence is more directly hormonal, strongest in older or hypogonadal men. Neither substitutes for diagnosing genuinely low testosterone.
What should a good product state on the label?
The species (Eurycoma longifolia), plant part (root), extract ratio (commonly 100:1 water extract) or standardization (e.g., eurycomanone percentage), and a dose in the 100–400 mg/day range. "Longjack complex" inside a proprietary blend states none of that.
Is tongkat ali a steroid or banned in sport?
It is not a steroid and is not on the WADA prohibited list. Adulterated products are the risk for tested athletes — another reason for third-party-tested brands.
