What these products are
Prostate formulas — saw palmetto leading, beta-sitosterol, pygeum, pumpkin seed and zinc behind — sold against night-time urination, weak stream and "prostate health." What they cannot do
- Shrink the prostate or outperform placebo reliably. CAMUS and STEP — the large NIH-funded saw palmetto trials — found no benefit over placebo even at triple doses; Cochrane concurs.
- Substitute for the rule-outs. Urinary symptoms overlap between benign growth, infection and cancer — the symptom pages' red flags and a PSA conversation come first.
- Beat the drugs. Alpha-blockers work in days; 5-alpha-reductase inhibitors shrink glands over months — comparisons the aisle avoids.

What survived: the symptom score and exam a weak stream deserves (BPH is treatable; the checkup also rules out what capsules can't), saw palmetto's honest record — trial-negative at scale — and beta-sitosterol as the one botanical with modest positive urology trials at printed doses.
